Réjean Hébert, former Minister of Health and CIRANO researcher, wrote the chapter « Canada: Application of a Coordinated-Type Integration Model for Vulnerable Older People in Québec: The PRISMA Project » in the book titled « Handbook of Integrated Care ».

This chapter presents the PRISMA model and shows that it is feasible and efficacious to improve integration functionally without—or in spite of—structural integration and merging of organizations. It addresses the main challenges associated with this approach, including monitoring implementation, allocating adequate resources, adapting funding mechanisms, and institutionalizing the innovation, given the complexity of healthcare systems.
