We have developed an analysis tool to conduct incident/accident investigations in hospitals called DANAISS (Déclaration et ANalyse approfondie des Accidents et Incidents survenus dans le Système de Soins). This tool provides a structured methodology and 6 checklist sheets.

Structured methodology
This structured methodology helps to provide a structure for incident-accident analysis within the hospitals and to standardise data collected on undesirable events, their causes, contextual factors, and the management of the situation. It can help to evaluate and collect data on incidents and accidents occurring in hospitals and to create database at local and national level.
The structured methodology includes 10 chapters:
1. REPORT FORM (AH-223 incident/accident report);
2. A CONCEPTUAL FRAMEWORK for thorough analysis;
3. A STEP BY STEP METHOD to conduct a thorough investigation;
4. A list of TRIGGER CRITERIONS for thorough analysis;
5. ADVICE FOR THE COMPLETION OF THE THOROUGH ANALYSIS;
6. A description of the REPORT OF THOROUGH ANALYSIS;
7. A DATA ENTRY FORM OF RESULTS OF THOROUGH ANALYSIS;
8. USER GUIDE OF DATA ENTRY FORM;
9. APPENDICES;
10. TOOLBOX (tools, which aid in the completion of the analysis and checklist sheets).

Checklist sheets
The sheets have to simplify the trigger and the completion of the analysis. The analyst can always consult the structured methodology if needed.

  • Sheet 1 : list of trigger criterions
  • Sheet 2 : Steps to follow to organize the thorough analyses
  • Sheet 3 questions and advise for organizing meetings and perform
  • Sheet 4 : additional questions to sheet 3 for causes analysis
  • Sheet 5 : additional questions to sheet 3 for recovery phase analysis.
  • Sheet 6 : Advise to write the analysis report and to communicate the results of the analysis.
